Patients, please ensure your details and information is up to date when you arrive for your appointments this includes, next of kin and emergency contact details. This is the patients responsibility to notify us of any changes.


A person must be listed as your next of kin or emergency contact to be able to collect information on your behalf, this includes asking for appointment details, picking up scripts/referrals etc.


We are very strict with our confidentiality policies which are in line with the Privacy Act.
